Can you forward this, it's a medical form for the abseil.

 

First registration will be at 2pm. They will take 10 people every 20mins. We have about 25 people doing it so first 10 people will register at 2pm the next 10 at 2:20pm and the last at 2:40pm.

 

There are still spaces available if anyone wants to come in at the last minute. If they want to they can pay £30 just to do the abseil..

I have attached medical forms for your group to complete and bring along on Sunday.
